Milford Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. We also offer a dedicated memory support unit for residents with Alzheimer's or dementia. We accept Medicare, Medicaid, and most private insurances. Dr. Toledo graduated from medical school at Universidad Industrial de Santander in Colombia and did his residency in Internal Medicine at Geisinger Medical Center in Danville, PA. He is also currently certified with the American Board of Internal Medicine. In 2019, Dr. Toledo was named Physician of the Year at Bayhealth Kent Campus and has recently served as a member of their COVID-19 task force. In light of the potential harm the pandemic poses for nursing home residents in particular, Dr. Toledo Rivera is vigilant in ensuring the safety and health of Genesis Healthcare’s patients. Placing your loved one in a nursing facility can be overwhelming, and she strives to make that transition as smooth as possible for both the patient and family.
